Magnolia earned his ba at the University of California at San Diego followed by medical school at the University of Texas Health Science Center at Houston he completed his residency in Pediatrics at the national Naval Medical Center in Bethesda Maryland and a fellowship in Pediatric and Texas infectious diseases at the University of Texas Health Science Center 27 year career in the US Navy he joined cincinnatichildrens in 2006 as a clinical researcher he is the director for the center for vaccine research and the principal and alligator for a National Institutes of Health funded grant called the vaccine treatment and evaluation unit which evaluates new vaccines and Therapeutics in all areas of infectious diseases he has conducted numerous clinical trials and this year has been in charge of five different covid-19 vaccine vaccine trials conducted at Cincinnati Children's Hospital Medical Center has published over a hundred and twenty-five manuscript on various topics and infectious diseases and has presented his research both nationally and internationally on behalf of the Immaculate Heart of Mary Health advisory commission I'd like to welcome doctor Frank thank you very much Liz and that thank you for that nice introduction and so if you can let's see if you got it okay I can share it okay and get this right here okay so thank you very much for having me tonight and what I was going to do is just trying to give you a bit of an overview about covid-19 and and vaccines and then hopefully hit most of the high points that I've heard people asking questions about but I'd be happy to answer questions and anything else they're just as a background the first place to buy covid-19 so it's a novel coronavirus and what that means is that humans prior to 2019 had never been exposed to the virus was first detected in Wuhan China and just rip 2019 and now it's basically throughout the world and to the official name for it is SARS coronavirus to and stars mean severe acute respiratory distress syndrome coronavirus to you and that the stars is another Corona first we saw in the 1990s that had a big outbreak and then it basically just disappeared we still don't understand why it disappeared but it did people were hoping I was going to happen with covid-19 fortunate that hasn't I'm inside the disease caused by the virus has really come to be synonymous with covid-19 and what that means is that it was identified in 2019 some people have said that because it was the 19th coronavirus but that's not true is because he was identified into what's a 19 don't want to think people ask me a lot about about transmission of covid-19 and so I study that was done by the Centers for Disease Control CDC they studied a hundred 991 index cases so these are people with on covid and then they had at least one other person in the home what they found is the secondary spread so people that were exposed as index case is about 53% so out of a hundred people exposed 53 became infected and 75% I'm infected become sick within 5 days of exposure and that's really where we getting a lot of these numbers as far as with quarantine for the 10 days and not because if you haven't become Sick by 10 days you're not going to get sick and the rate of spread was equivalent across the age and race however women were less likely than men to be in a secondary infected so 50% of women versus 57% of men this light is looking at the cumulative I'm infection by age and so here this is the number of cases per hundred thousand people and this is time going from March of 2020 to November 2020 and so early on you can see here so these are the different age groups of 0 to 13 14 to 17 and on and so this here is greater than 80 years of age so initially so an April or so we're starting to see a lot of case United States the majority of cases were in people 80 years of age and above what we see now is that the highest rate of infection in the United States and this is as in November but it's really stayed the same as in people 18 to 49 years of age where is it and so that's really why it's important that we are having young adults vaccinated and so what can we do to try to prevent covid-19 of well frequent hand-washing works really well and and face protection so our masks and The Mask has been shown to be 85% protective in preventing transmission so just by putting on a mask and be really very good about doing that 85% protective social distancing so works so there's been a lot of stuff is from the CDC is talking about 6 feet social distancing but there's been some more recent data to show that 3ft works just as well as 60 and I'm from a school perspective that makes it a lot easier to be have kids go back to school if you only have to be separated by 3 ft it's about the social distancing and that I am face Protection work well but the problem with that is it have to be done all the time and people are tired of getting tired of it there and so they're getting a little more relaxed as far as using this precaution and that's why I really in my mind is that vaccines offer the long-term protection that we need so this light is looking at the different vaccines do we have in testing in the United States at War avail well some of them with their emergency use application authorization and so the platform this means two different ways that we used as far as to make vaccine so this is a nucleic acid of the MRNA vaccine the viral bacterial I know it's an adenovirus Vector vaccine and in the protein subunit which is closer to what we're typically have had for vaccines and then the developer for the MRNA vaccine is the Pfizer moderna vs. Johnson & Johnson or Johnson and asters cover the bio factor and the novavax sanofi has a protein subunit vaccine but it's probably not going to end up being tested the United States because they got kind of a late start and there's now going to be all these ones that I just mentioned it available United States do they really be able to do a phase 3 study in the United States so the nucleic acid vaccines have been approved for the Emergency use authorization as has been Johnson & Johnson and AstraZeneca has presented there. add to the FDA and they're waiting to have a decision made about being granted the emergency use authorization novavax also has now finished their phase 3 and are preparing the information to submit for a Annie way the simple things here is that just to go over some of the things that are untrue about covid-19 is it the vaccines do not make you infertile this is been something that has been a lot of on the internet I've seen enough but it's just not true that the scientific basis of where it was a bat on is that if you look at the spike protein and that's where that's the protein that the virus uses to attach there's four amino acids that are the same as in a protein in the placenta that used to Anchor or hook the placenta to the uterine wall and so people saying you know what this is shows you that this is going to be problem as if the antibodies are going to work against the placenta and it's going to make people infertile it's just not true the vaccines do not modify your DNA the MRNA actually did not even get into the has nucleus and that's the part of the cell for our DNA is so that for the MRNA vaccines didn't even get into the nucleus so they can't Incorporated the DNA for the adenovirus vector vaccines they actually do start in the nucleus but then they get Tran waited to the MRNA the messenger RNA quickly and put into our cytoplasm which is all the rest of the cell outside the nucleus and then then the MRNA can't get back into the nucleus so again even the adenovirus back to they do not change our DNA these viruses have been engineered in such a way that they can't grow either so that they're their job is to bring this bike protein gene into our cell and then they basically died and I'll go over that in another couple Slide the vaccines not cause you to test positive on PCR test PCR me the plumbers Chain Reaction which is the test that we're doing on the nasal swab if you test positive on a PCR that means you've been infected with the vaccines do not give you covid were using just pieces of the virus genes of to make the the different vaccines there are some vaccines that are called live attenuated vaccines where some countries are testing those were not using those those are the whole Koba. Have been mutated or change so they will stimulate an immune response but it don't make us sick but we have not use those vaccines United States and the covid-19 scenes they will not protect you against Blue unfortunately and the flu vaccine is protect you against covid-19 and one of the other things is that people ask a lot is that I've had covid-19 to get a vaccine still in the answer's yes and the reason I say that is because what we see is that even the people that have had covid-19 they got to be and I still get a very nice boots in their immune response so we really are advocating everyone to get a vaccine when it's available I'm so here just a couple definitions that are kind of important for people to understand because it helps them judge the data or interpret the data has been put in the the media about the vaccine so the first thing is infection and from an infectious disease Campo we met by infection is that you're able to detect the or isn't your body but then the other thing is disease and so disease means you are infected and you have symptoms so I get a positive PCR on you plus you're having a headache are you having fevers you're having chills are we having cough and then the other thing is is moderate-to-severe disease and what that means is that you're getting hospitalized or worse in the ICU or dying from the infection so I would love to have a vaccine that prevented all covid symptoms but what I really want to do is to prevent the cases that result in hospitalization and so if you look at that in that context will go over a minute though but in that context all of the vaccines that have available through the eua are 85% or higher protection against hospitalization ICU admission or death to this light is looking at the stars coronavirus in covid-19 and hear this part here is the spike protein and then if you blow it up a little bit here this part here is What's called the receptor binding domain or RBD and their receptor binding domain is the part of the of the spike protein that binds to our cells and so viruses what has to happen for the viruses to live is they have to find a cell and to be able to get into the cell and use Excel Machinery to be able to reproduce cannot reproduce by themselves and so that the virus uses this Spike protein to hook onto our cells and then get into SAR ourselves and then replicate and make new virus so this is a car I'm looking at the MRNA vaccine and so here what it is that this is the gene if I can make my court this is a gene for the spike protein and so and this here is a lipid or fat nanoparticles small particles of fat that go around the MRNA and the reason we had to do that is if you just give somebody your DNA naked DNA there's no covering on it it gets chewed up before ever gets into the south end so there's a couple points there one is that's what we had to put the lipid on there but too as far as people say I'm worried about the long-term effects what happens that we have chemicals in our body are called arness's that are these are enzymes that degrade virtue up MRSA and it and so that's why we had to put this lipid nanoparticle around it now once to the cells and take up the Gene and what happens is that this mRNA that uses our own cellular Machinery up what's called a TRNA of the transfer RNA to make the spike protein and then we put the spike protein on the outside of our cell and telling our immune system to make antibodies to this Spike protein if you were infected with Coban what would happen is it as the virus is making new copies of itself our body is also trying to make immune response against it and it would do the same thing so when people that are infected with Kobe. Get on the outside of your immune cells this Spike protein and is stimulating our immune system so what the vaccine is doing is simulating an infection and giving you an immune response without you getting sick this isn't looking at the adenovirus vector vaccine so the AstraZeneca and the Johnson & Johnson vaccine and so here this is the adenovirus and adenovirus in the common virus that we're all exposed to it typically causes a cold this adenovirus that they use the ones they use for both the Johnson and Johnson and the AstraZeneca has been modified so it cannot replicate a cat growing us it's only job is to bring in that Spike protein gene into so here what happened is it our cells engulf the virus and then the virus let's go of its DNA makes it to the RNA and then just like I showed you in the last slide then is it that mRNA is transcribed into the spike protein and again is put onto the outside of the cell so the ways the spike protein Gene gets into the cell is the different from the MRNA vaccines versus the adenovirus vector vaccines but in the end the result is the same as that you're putting this Spike protein on the outside of our cells I'm to stimulate our immune system so now with that just to go through a little a couple slides on each of the vaccines that are available to just kind of show you the data so that you can understand a bit more about the vaccine so this is looking at the systemic so the total body kind of side effects and a lot of people don't have any side effects but these are the common ones people have to have the common ones have been fatigue and headache and muscle pain fever has actually been unusual around I'm 3 to 5% on the whole one so this here is the 16 to 55 year old hears over 55 years of age the top Parts here dos1 the bottom Parts here dos2 so you compare here dos wanted those two for the younger just wanted those two for the older and then you can compare across this way dose one for the younger versus those two for the older so one of the things that we've seen with visor and moderna and AstraZeneca and Jansen Jansen is that the there's a higher rate of side effects in the younger people than older people the other thing that we've seen with visor and moderna is that there's a higher rate of side effects in the first dose compared to Second do so here this is fatigue in the first dose of fatigue and the second dose so instead of being 47% it was like 59 per-cent and so for headaches instead of being 42 / has 52% so that's one thing we're seeing is that for the for the MRNA vaccine that so the Pfizer and moderna we're seeing more side effects with the second dose in the first and we're also seeing more side effects in the younger the older when I tell the young nurses that are working with me I said let's cuz when I get out of bed in the morning something hurts anyway so I just don't even think about that being a symptom but we are finding that the younger people are having more symptoms to say things that we really can't explain is it with the Johnson & Johnson and AstraZeneca there's more symptoms with the first vaccine than the second vaccine I don't know one of the vagaries of the difference but the other thing is that with these vaccine side the green is mild and the blue is moderate red is severe you see that really no one had severe and so that the side effects if you're going to get them they start typically between play form 48 hours after the vaccine I would say pretty close to twenty-four hours and that they last a day or two I can tell you what the first dose I I had them Eternal vaccine the first dose I rolled over on my got my right arm is Arie Bluebird on my arm of the midnight it woke me up and that that but other than that it wasn't big deal the second dose Tylenol with my friend for a days I did not feel good for a day but then I was fine I mean I had some chills and fever and Lakeside basically felt like I had the flu for 24 hours and then I was back to fine so this light here is looking at the results of the Pfizer vaccine and so this is the percentage of people here on the vertical axis the percentage of people that we're getting infected and this is time and so that the blue curve here is a number of cases are happening in Placebo recipients and this is the number of cases are happening in vaccines and the white space in between these two curves is the number of cases that were prevented by people being vaccinated the Pfizer and moderna vaccines both were developed as two dose vaccine so it's hard to say much what the effect is after one dose but that's what we're trying to show with this cut out here is that so even about 12 days 10 to 12 days after one dose you're already starting to see separation of the curb that puts a vaccine vs placebo and is estimated that one dose of the vaccine for both by Zurn and modern about the same about 62% protective efficacy of one dose versus the 95% with two doses so then with the moderna vaccine here again of looking at a systemic side effects so this is placebos and dose 1 and 2 and this is the vaccines that does one and two and so you be here again the most frequent head symptoms are fatigue and headache and then muscle pain and just like you saw with the Pfizer is that it's more common to have side effects of the second dose in the first so this is for headache second dose first dose fatigue second dose first dose myalgia or muscle pain second dose first dose and here's the results of the efficacy the protective effect with the moderna and on will you can almost superimpose this on top of the Pfizer it's really been remarkable when we started these vaccine trials we were hoping that we would have one vaccine that would give it at least 60% efficacy we now have three things that have the EU 80 mercius authorization that are 85% + 4 from preventing hospitalization in the end like I think we'll have another too soon that will be in the same range stove but here just to give contacts again this is the number of this is percentage of people infected overtime Placebo vaccine and the distance the the space in between here's a number of cases in a method and again their numbers came out right around 60 percent efficacy after one dose which is what they're trying to show with these arrows here is that there's some separation between the two curves then moving to the AstraZeneca very similar kind of to hear other than as I was mentioning as far as that the side effects are more common in the first dose in the second dose for this but if you look otherwise as far as this is 18 to 55 Euros is 59 to 60 this is over 70 year olds and so this is fatigue you can see that as people get older this the complaint of the fatigue decreases fever decreases headache two creases all the symptoms as people getting older decreasing in reporting this is a similar kind of thing as I was showing you for the moderna and the Pfizer and here's the efficacy for the masses Anaconda so this is the percentage of people infected and here is the curb for the people that received vaccine excuse me Placebo here's the and their Placebo they use them meningococcal vaccine and that here's the vaccinated group these shaded areas here is that confidence intervals so it says that this is the actual number but the with the statistical significance could be somewhere up high as here as low as here but regardless you see there's a fairly good separation between the two curves again showing that the vaccine it's preventing a lot of disease and then for that the Johnson & Johnson similar kind of thing of looking at the side effects and that hear what you're seeing is looking at the low-dose and high-dose placebos so as though they giving a higher dose they were getting more side effects but they chose is actually using a lower dose there lower doses they were going forward but very similar kind of thinks it with the Seneca and Jansen as far as having first dose having more symptoms still on everybody the younger had more symptoms than the older and hears them the similar graph but looking at the Johnson Johnson or Jansen vaccine oops scuse me and again here's the placebo curve here's the ad no viral vector or the vaccine curve and the separation between the two curves showing the protective effect of the vaccine so in summer is far as with the side effects is that me people had minimal or no symptoms and if you going to have the Simpsons has matching the common or headache fatigue and muscle joint ache chills and fever on usual around 3 to 4% and the symptoms typically start about one or two days after the vaccine at last about one to two days so how are we able to use these vaccines now it's because it was called an emergency use authorization and what that is it's in cases of declared medical emergency such as pandemic Food and Drug Administration the FDA has the authority to allow the use of medical products so medicines vaccines that are still being tested to be able to be used as long as there are not effective treatment or preventive measures that are licensed in for the covid-19. Have anything is license that is effective for treatment or preventative so that's how the vaccines were able to be used with the emergency use authorization Ford the vaccines were able to be used for emergency use authorization there had to be at least two months of safety data on the back scenes so that the FDA could review those and the reason the two months was chosen is because we've shown and in a number previous studies is that if you don't see an adverse event or side effect by 2 months you almost never see a side effect and so that's that's why they they chose the two months safety data the vaccines given through the eua many of you hopefully know is basically like going to the doctor's office to get a vaccine but we are supposed to be handing people a fact sheet that contains information on the product safety and the bail ball ternative products with the covid-19 there really it's the vaccines in there really aren't any other products other than the vaccine and we're still collecting information about side effects and that there's a it's called be safe with the at the CDC your settings Disease Control where a do people to write if they are having side effects because the CDC is collecting information on that you could also submitted through this called bears in the back scene adverse event reporting system with the CDC so then going to a little bit about children is that Pfizer has completed studies and Adolescence there were two stages and adolescents the first was 16 to 17 year old the others 12 to 15 year olds the results from the 16 to 17 year olds were available to time of the initial emergency use authorization request by Pfizer and so the FDA did give the approval to have visor on be able to be used for the easy way from 16 and above and so the FDA can only give the e-way of have data and so Pfizer at that time was the only company that had data so that's why Pfizer is the only vaccine on this available under 18 years of age on the Pfizer also I'm now has results for the 12 15 year olds and we just finished the manuscript that's going to be submitted this week I think and that looks like New England Journal of Medicine is going to publish it and the immune responses were Courier action to 12 to 15 year olds compared to 16 25 year old so that not only were they not only equivalent they're actually better give him the same dose in 12 to 15 year olds in the 16 to 25 year olds also I would say protection was equivalent to adult visor came out and said it was a hundred percent effective I don't like they were say that because all you have to do is have one case in the vaccine is no longer 100% but they're so far they were 18 cases of Coke Placebo in the 12 to 15 year old and no cases in the vaccine recipients and and we are now waiting for the decision by the CDC to lower the age of the e-way to 12 years of age I think that will happen in May so I think by May we'll have a vaccine that the 12 and above will be able to receive one of the things that impressed me with that the data from the 12 to 15 year olds is that for the for the ear play for the adults we had 30,000 people in the adult studies and that's how many we needed we figured to be able to get enough cases to show that the vaccine was effective we only had three thousand adolescent the Adolescent study was actually designed for what's called immunologic abridging just to compare the immune response in the children to the adults and the design was to say is that if the immune response was the same what we as I mentioned it before we found is actually was better in the in the younger children are a younger adolescence and in the adult then we would make the extrapolation of the assumption that the protection with equivalent we did not deserve find a 12 to 15 year old to look for protection but there was so much covid in the young adolescent that we were able to show efficacy with only 3000 Kids versus 30,000 adults so to me this is another reason to show why it's it's so important to be immunizing our children so we now started studies and younger children multiple vaccines are being evaluated so Pfizer has study moderna has a study novavax has study and Johnson & Johnson has studies so that we're going to eat we have a number of vaccines being looked at for children I were doing a descending age groups of First 5 to 11 years and 4 years and 6 to 24 months and the results from these studies likely will be available around the end of 2021 to early 2022 so I think that it's possible for the 5 to 11 year olds that the available little bit sooner because we are starting with those that age group first on so it's possible that you could have an emergency use authorization decrease to even five years of age by September I think it's probably going to be more likely around December but it's still possible he even has before the 2122 school year starts that we would have a vaccine available for children as young as five I'm so where do I come to look at the future is that the covid-19 vaccines have had a good safety profile and protection against disease we now have three vaccines available through the eua and I think we'll have two more soon I am before the 2122 school year covid vaccines I'm confident will be available for children 12 years of age and older and probably going to be a little bit longer for the children below 12 so just in clothes is this is one of the slides from the CDC I downloaded a couple days ago of their data on vaccination and so this is the rate of vaccination age groups from 0 to 100% And then the different age groups here and so over here gives you that kind of the aggregate of the different age groups and so you got to really kind of ignore the under-18 because the only people that can get it are 16 and above soap but if you look at if you stay at 18 years of age 18 to 29 we only have 32% of even had one dose and so we don't get above 50% until we get to 50 years of age and so we still have some room in this younger age group to get vaccinated and a few couple this with the slide that I showed you earlier as far as at the highest infection rate is in this group the highest rate is 18 to 49 and the lowest immunization Ray is 18 to 49 that's the group that we really need the target until we have vaccines available for the children so that we can include them in the vaccine group what we really need to Target is our our young adults right now and I think that's it soul is all stop sharing give it back to you and I'll be happy to answer any questions I can now that you have you see I know there were a few that popped up in the chair okay I can look and see if you want in the sea I think I have everyone muted so if you agree doesn't your chart include us as reported on Bears so the Adverse Events that I was showing you their those are with the clinical trials and we did not have deaths in the clinical trials and so that one of the things that happens then with when you start to do large-scale immunization so giving something to millions of people if you say just in Cincinnati everyday what is going to die of cancer someone's going to die of a stroke somebody's going to die of a heart attack this regardless if there is a cobra and a man that's just going to happen because people get sick and so the one of the things is trying than to tease out the difference between causation and Association so Association just means that things are happening at the same time where is causation is that what causes this want to happen and so with the Johnson Johnson vaccine that was where they had six cases the last I've seen there may be one or two more of that but I've having blood clots out of C a million people that had received vaccine so the incidence is 1 in 1.1 million people literally the likelihood of getting hit by lightning is higher and so that it's there is a very rare side effect it looks like may be caused by the vaccine and that's being added to the information sheet about Johnson & Johnson but it was looked at as far as being very steal the benefit of the vaccine far outweighed the very rare risk so that is so Desu at the end that's what I was mentioning as far as that Bears so the vaccine adverse adverse event Reporting System doesn't ask you to have causation they just state report it and that so that the people at the CDC can start to look at it and see if they're seeing any patterns or any things that would be worth I'm so you will get reports of people having death I mean I was told that one of the things that so Hank Aaron famous baseball player got covid-19 and dyed short I'm afterwards but it was not related to the Kovach seen he had underlined comorbidities that he died from but people were saying to see he was okay and got the code vaccine and he died and so that's one of things I can say is it's a it's a hard message to try to discuss with people but that really a kind of may be back just in vaccine avoidance in general is it if you look at it for a long time people are talking about measles causing autism and so measles vaccine is associated with autism because 95% of kids get measles vaccine so measles vaccine is going to be associated with everything but if you look at the rate of Oz is the number of cases for thousand of autism in the kids that got measles vaccine vs the kids did not get measles vaccine it's identical totally the same so that the measles vaccine did not cause the autism and that's one of things that were keeping a very close watch on side effects with the covid-19 at this point there's this one says to the very rare event but it was looked at closely and really still thought that the safety and the benefit was far outweighed the the risk and was trials and younger Champion Flip Factory 10% will data for the trials and younger children be influenced by but she would mean as far as with the data been try so the as far as the children becoming infected we're still having a lot of children becoming infected and that while it's true that the likely severe disease in children is lower than it is in adults is not 0 we had over 300 children the United States die of covid-19 healthy normal kids that covid-19 we had now I think 14000 children have been hospitalized for Kobe Bryant over three and a half million children in the United States have been infected with Coben so there's a lot of cases of covid-19 children as far as if you're saying the data with efficacy I really don't think it has anything to do with that if anything what we seen in other viral infections that specific like flu and that is it immunizing the children decreases the race in adults not the other way around because the kids are very happy to bring home their colds or flu or whatever kind of stuff and sneeze and cough all over you and and give you their cold I mean I was as Liz mentioned I was in the Navy for 27 years so that I would from Duty station Duty station and I knew when I moved the new do station I was going to be sick for the first few months because the kids were going to get me what other new ever knew by Iris that they had at that at that Duty station give me so so based on that statement my question is is the real is the is underlined greater importance of the children being vaccinated to help protect the older Generations getting I think there's two reasons to vaccinate so I tied it raises the direct effect of protecting the children and that and so that because as though I think you were some was asking them I was asking as far as when they rarely get sick and and the symptoms are so very mild that's all I can say the 300 compared to close to a half a million adults dying from the disease is actually a very small number but these these were healthy normal kids before they got covid-19. Predict which child is the one that's going to get really sick so there is the direct benefit and then but it's as you're asking is that the indirect fact is at least as important if not more important to try to decrease transmission to adults then if let's say and I don't I don't I mean you shared a lot of data which I appreciate but it was a lot to take in so I apologize for not remembering all of it but it is going to give you a test I would fail okay but but but in terms of that if we get to a point where 50-60 70% of the 18 + generation or age group is vaccinated you know where is parents how do we how do we make sense of looking at this from a very thoughtful standpoint and not saying we're just going to not do it because we're scared but also not just going to run out and do it because you know we've had you know two months of testing right I think there's a middle ground there as parents that we're trying to figure out and say you know this is the first vaccine that or Cheney this is the first MMR and a treatment that the FDA has ever approved and it's been done an emergency basis I don't think a volume of data is is enough to secure some parents concern not to say that I'm anti-vax I'm not at all but I think there is a a considerate position in the middle that says wait a second if the mortality rate really low for the children and most of the adults now that are really a death at risk for mortality from this are vaccinated where's that wears the really pressing point for me as a parent to think that I have to run out and do this right so I appreciate your question and Let Die and I feel like as a physician my job is to to provide information so that parents can make some good decisions and that I think that there's a myriad of reasons why people make decisions and that so like I said I I kind of icy it is my job to try to prevent present the information as best as I know it I think that's what you're asking is it makes sense as far as it went to things as far as that with the MRNA vaccines to add Navarro Vector vaccine while they haven't been used in a mass vaccine before they're not brand new and so for the MRNA vaccines that they actually were started for as a treatment for cancer because on cancer cells sweet have these abnormal proteins are antigens on the surface of the cancer cells and so they're they're trying to use the vaccines as an adjunct to treat cancer time to recognize the foreign cells in to kill them for the adenovirus vectored vaccines those were actually developed specifically for pandemic and it was has it been used for Ebola and found that they were very effective in quelling the a bowl of epidemic in Western Africa and so but as far as you know if you look at it as a corollary I think that best when I can think of is far as looking at influenza is it during a bad flu year there's about a hundred and thirty hundred forty children that died from flu that's it's horrible still but it's about one-third or one whatever little maybe more than a half as far as that of what I have died children died so far from Corbin so you know they're there is that that direct risk to the child you know we don't know they're still thinks we're learning about kobita may it it may be that we got a good immune fonts for the population and is able to knock the virus down and we don't see continued transmission you know that the vaccine at 6 months and that's really as far as the out the data we have at this point has continued to so incredible protection is that some people have misinterpreted the data to say that the vaccine only works for 6 months what visor was saying and moderna has the same information that at 6 months is still 90 2% protective so that I would look at it and say that the vaccine is still incredibly protective even a six-month so as far as for the the kids the the the MRNA is broken down very quickly very quickly about one to two days it's degraded is gone did the immune response that the MRNA and the adenovirus vectored vaccines are stimulating is basically what would be stimulated if you were infected with the virus it it's using the same kind of process except for its doing it without us having to have the infection the the novavax vaccine is a protein vaccine and still more like the typical flu vaccines would give on a yearly basis and so it has a little bit different stimulation of the immune response but the still looks like it's being very effective but they're the MRNA vaccines and the Admiral bahroo but after vaccines are really very closely mimicking the immune response that our body would make to us if we were infected with virus so you know anytime we're doing something new I totally understand it's scary and that you know we are watching things closely we have not seen any long-term text that the back the virus is the virus for the adenovirus in the MRNA really aren't around for a long time so there's really not a good scientific reason to see why there would be a long-term effect but you know we are monitoring got in the in the clinical trials them too so I think at this point what I would say is that I don't think Kobe going to go away whether or not we're going to need boosters I don't know I think we're going to we're going to have to check that over time to see how the vaccine continues to hold up and that's one of the things we're doing at the clinical trials but you know 17% of our population patient is under 18 years of age and so that if we don't immunize kids we're going to have a big percentage of our population that's going to be out there to be carrying on the transmission of the virus so I mean you know I think a lot of these other Canterbury kind of similar that that they've as far as the to the they're very similar kind of context is far why now why get the vaccine so hopefully you know just trying to give you that idea as far as with the aborted fetal cells nut it's was what they used is that this is there's nothing there are no no fetal cells in the vaccine viruses need a cell line to be able to grow in so that for the MRNA vaccine there is no fetal tissue there is nothing time to spend used to make an mRNA vaccine that's if you're testing the the efficacy of the MRNA vaccine you need to have a cell line the buyers will grow in with the vaccine has nothing far as with fetal tissue for the adenovirus vector vaccine again there's no fetal tissue in the vaccine but the virus they used fetal kidney cells that's the cell line that was collected in the 1960s to grow the virus but that's that's the only place where there's any sort of tissue just to be able to grow the virus has been purified out I meant it's just a virus that is being used for the Admiral viral for the MRNA there's no tissue. Negative so you know as far as that as far as with the infertility we have not seen anything there epigenetic effects I'm in the vaccines mRNA and the did not get into our genome it does not incorporate is not stay around so that there's not really anything of where we should be seeing anything with the genetic chain that and so that that's really kind of the best I could tell you there that there's really no scientific reason of why it should be associating because the the the MRNA cannot incorporated into our genome and that and that's really what these vaccines end up being as far as the genes for the spike protein that the and you know I think what part of the things is that again I could say as far as that understand of people's being concerned is that the way that I would look at it thanks 16 months ago or so whatever is getting to be now that we just started hearing about this virus and so it was a a new thing we rapidly saw how dangerous it was how many people are dying from a we are seeing that is not gone in a one of the slides that took out just for brevity is that looking at in flux influx of cases is that in November December we had a key increase the number of cases because people were traveling they're going to see their families around the holidays and we had a big outbreak again of Kobe in Europe were there seeing it again India you probably heard as far as having a horrible problem now is so that the virus is not gone and that you know we're trying to get it under control I think we're doing much better that the hope is that will see that the the vaccine not only does prevent disease but prevents transmission will be seen with a study that came out of Israel is that the vaccine to fight it was devised vaccine early but you just that's where the daters from that had 90% decrease in transmission and so if we see another other data that proves that that would give more ability for public health people to say that once you're fully vaccinated you really don't need a mask because and that's why if people ask me why are we wearing masks still is because we know that the vaccine prevents you from having disease we think it prevents from having transmission but we're really needed to collect some more information on the transmission to make sure we know that for sure and like I said we do have that one study from Israel which was extremely what is extremely hardening the vaccine does not have hotels in it it was grown in kidney cells it's purified out and so that's kind of the best I can tell you that and I think that I got to the bottom of the questions channel is if there's other questions or if I put everybody to sleep by now or whatever you know that the time trials in the children have you seen the same Effexor side effects side effects actually if you the kids have done almost identical and that the and the thing has been so we may well be able to give a lower dose in kids because their immune response works really well that's why I like being a pediatrician is because kids are growing organisms and that they're healthy and if we give him a little bit of a boost that they get back to normal where is when you're getting old decaying things like me that you know we need we need a little more help and so but the at the side effects of eating really mirrored in the older younger adults and even older dog so that nothing different nothing special and it is really been the same kind of thing is that with the headaches the fatigue and the muscle aches is lasted typically a one or two days and then they're back. Okay and it's been really rare that we seen any of the kids that had to stay home the only reason we had kids that had to stay home is because they got a fever and then if they got a fever then the school says you can't go to school cuz you got a fever we don't know you don't have covid and so that it was really weird trying to but they had the vaccine that's what caused him to have the fever but I can understand the school doesn't want to have people that potentially are infectious so that's really the only one that reason I know children missed any school was because of having a fever for a day or two and that were thought to potentially have coping I was also going to ask have you had a lot of people volunteer for children to get the vaccine or so we work as you mentioned before is it at Cincinnati Children's one of nine sides in the country was called the btu the vaccine treatment evaluation unit and so I've talked to my colleagues at different places across the country and evolve had similar experiences that we've had about 3,000 families that have asked to be on an interest list for vaccines right now we have completed them at least initial studies for 12 to 15 year olds and 16 to 17 year olds we're now moving down into the below 12 and so we're just finishing up that the studies are we looking at the doses to see if we can if we need to use the same dose or we can use a lower dose just because as kids get smaller they may not need the same dose to stimulate the immune system and then once that we're finished with that and then we'll be starting a bigger study and the kids 5 to 11 and then going down to 2 to 4 and then six months to two years and so that's will be then calling the families that have been on the is this but yeah we had about 3,000 and then people at other universities that I've talked to had very similar kind of responses thank you dr.
